Append-only retention

Audit events are appended rather than overwritten. Prior review history remains part of the institutional record.

Mutation recording

Review state changes, route changes, reviewer notes, blocked transitions, overrides, and policy evaluations are retained as governance events.

Transition accountability

A transition records previous state, next state, previous route, next route, actor, mutation source, and governance reason where available.

Policy override recording

Policy override is not silent. It is recorded as its own event and remains connected to reviewer attribution and decision context.

Institutional continuity

Continuity preserves how authority, policy, and operational context shaped a request over time.

This doctrine describes public governance posture. It does not expose internal review surfaces, tokens, or operational request records.
